You've probably heard that health records will soon flow easily to apps, with AI assistants to help.

In the US, the government announced pledges in July 2025 for apps that use "conversational AI assistants" and tools to "kill the clipboard."

So is it really that easy? We checked 198 countries and territories.

Pieces or the whole picture

Sam's record is in pieces: a doctor, a hospital, a pharmacy, an insurer, an app.

Aino logs in to one national service, OmaKanta, and sees her visit notes, lab results and prescriptions in one place. About 6 in 10 people in Finland used it in 2025.

With one service, the pieces sit together. You can see them all. The doctors who treat you can see them too; outside your region, only with your permission. With several portals, the pieces sit in different places, and no one has the whole picture in one place.

Which one sounds like you?

Finland: about 6 in 10 people used the one national service, OmaKanta, in 2025. One bar for each tenth of everyone living there.
US: 59% of US adults had online records or patient portals in more than one place in 2024. One bar for each percent of adults.

Not the same measure: Finland's figure counts everyone living there who used the one service; the US figure counts adults whose records sit in several places. Our Finland page says OmaKanta shows visit notes, lab and imaging results, vaccines and prescriptions from public and private care. Records go back only to when each provider joined, in 2013 or later.

Kanta says 3.3 million people used OmaKanta in 2025, 41.1 million times in all (in Finnish: "Vuonna 2025 OmaKantaa käytti 3,3 miljoonaa henkilöä yhteensä 41,1 miljoonaa kertaa."). Kanta counts people, not logins. Statistics Finland counts 5,652,881 people living in Finland on December 31, 2025. 3.3 million of 5,652,881 is about 58%, or about 6 in 10. Kanta's English page stops at 2024. The Finnish page, updated June 10, 2026, has 2025, so we use it.

The US figures are from a national survey of adults, one adult per household, chosen at random, in 2024; people who did not answer are left out. 59% of all US adults had online records or portals with more than one doctor, hospital, insurer, lab or pharmacy, and 7% used an app to combine them. 65% were offered and used an online record or patient portal in the past year. Finland's share counts everyone living there, children included, who used one service; the US share counts adults with records in several places. The two are different measures, so read them side by side, not as a contest.

We reran the ranking 10,000 times. Each time, we added a random nudge to every score, typically about 5 points, and shifted the weights. The size comes from how much a second agent disagreed with us when it scored without seeing our scores. The US landed between 45th and 80th in 90% of runs. This does not test our choice of categories or who holds the keys.

The details

AI agents read laws and other public sources and suggested each score against a written rulebook. A second AI agent checked each country. The four authors ruled on every finding that check raised.

Not by our measure. Sharing records is one thing; deciding where they go is another. The typical country scores 39 out of 100, and no country meets our strict bar for putting you in charge.

But some do much better. Finland scores 71 out of 100. And in the US, records are moving more between hospitals.

The pledges are real. So are the gaps. Our view: an AI tool can only work with the record it's given.

In a national survey, 76% of US hospitals did all four key ways of sharing records in 2025, up from 70% in 2023 (ASTP/ONC).

The last line is our view, not a finding. It matches what SuperTruth sells (paper, section 12 and disclosures).

Read the detail

Some health systems are short of doctors and nurses and slow to treat. People who can pay go private, and if private clinics keep their own records, the person's record splits in two. This table shows four public measures of that chain for each country. It changes no score and it is not a ranking. Chile shows all four signs of strain. Mexico and South Africa show three of the four; no wait figure is published for either, so the fourth is unknown. Waits are reported for only 19 of 198 countries and territories. OECD ranks compare a country with the 38 members of the OECD (the Organisation for Economic Co-operation and Development), mostly high-income countries.

Your health record is about you. But it isn’t really yours.

A health record follows you through every part of care: the family doctor, the hospital, the lab, the pharmacy, the insurer. Each of them sees its own part, and you may see the least of it. Your health data is shared, stored and sold, often without asking you.

AI raises the stakes. Tools that read records to suggest diagnoses, summarize notes or sort patients are only as good as the record they read. Feed an AI a wrong or missing record, and it can suggest the wrong care. This index holds up a mirror: who holds your record, country by country, and how much say you have.

How we score 198 countries and territories

How we used AI

Couldn't the AI just make things up? It could, so we built checks around it. SuperTruth's work is checking data before an AI acts on it. We held our own agents to the same rule: no score without a source.

What the agents did. AI research agents built on Anthropic's Claude (Claude Opus 5.5, run through Claude Code, September 30 to October 2, 2026) read public sources: law texts, government and regulator pages, international bodies, academic papers and news, plus some law firm, vendor and blog pages. For the countries added after the first 43, they searched in each country's own languages, official sources first. They scored each country in eight categories against our written scoring rules and cited the pages behind each score. A second agent session checked each country: it rechecked the links and the publishers and compared each score with the rule for its situation. Agents also ran the consistency audit, collected and first reviewed the real cases, ran the blind re-scores, and drafted the working paper and its internal reviews.

What people did. The authors directed the research and rule on every audit finding and every proposed score change, each recorded with its evidence in our public list of score changes. A few rulings are still open; that list shows which. The weights are written in our scoring rules and published with the data.

What is plain arithmetic. Once the eight scores are set, the overall score is a weighted average with the published weights, like a calculator: the same scores in always give the same answer out. Anyone can rerun it from the open data. For rank ranges we use chance on purpose: 10,000 simulated re-scorings, varying both scores and weights, give each rank its likely range.

The checks, with their numbers
  • Cited pages. The rules say every number and law name must come from a page the agent opened. A page that opens shows it exists, not that it says what our summary says. Apart from the blind re-scores, no check compares a summary with its source.
  • Quotes. 117 quotes from 41 outside documents in our illustrated stories are checked word for word against stored copies of their pages each time the site is built.
  • Testing the checks. We planted mistakes for two of our checkers to find. The checker of the real cases found all 8 rule breaks we planted. The checker of the blind re-scores reads the re-scoring agents' records to make sure they never saw our scores. We planted 13 places where they had seen our scores, and it found all 13. In our separate study of the 50 states and DC, not yet published, a checker built for that study caught 24 of the 24 errors we planted.
  • Blind re-scores. In two studies whose plans were written and locked before any scoring, agents that could not see our scores re-scored samples of single category scores: 51 of 58 and 94 of 108 came within 10 points of ours at the time (agreement 0.82 and 0.84 on a scale where 1 means full agreement). The re-scoring agents are the same model family as ours, and no person has re-scored the index.
  • Keeping the agents on course. Written scoring rules that set a score range for common legal situations; every figure computed from one version of the data, with a build that refuses or leaves out figures computed on any other; every ruled score change logged with its evidence.

The checks make errors less likely. They do not rule them out. If you find one, tell us.

Evidence grade (DTI™)

Every score also carries a grade for the evidence behind it, using the eight dimensions of SuperTruth's Data Trust Index (DTI™): provenance (primary sources count most), open availability, recency, completeness, corroboration, verification, breadth of sources, and stability (whether our audit found the same legal situation scored differently elsewhere). DTI was built to grade health records; here it grades our evidence. It never changes a score. Tiers: Platinum 90 and up, Gold 80 to 89, Silver 70 to 79, Bronze 55 to 69. Our own rule on top of DTI: evidence with no primary source (law, government, regulator) is never labeled above Silver, and neither is a country where fewer than 4 of its 8 categories cite one.

Terms used

  • OECD: The Organisation for Economic Co-operation and Development: 38 mostly high-income countries that publish comparable health statistics.
  • WHO: The World Health Organization, the United Nations agency for health. It publishes country data on health workers and spending.
  • EHDS: The European Health Data Space: an EU law (Regulation 2025/327) that gives people in the EU access to their health data across borders. It applies from March 2027, with patient summaries and e-prescriptions from March 2029.
  • GDPR: The General Data Protection Regulation: the EU law on personal data. Health data is a special category, and people have a right to a copy of their data.
  • HIPAA: The Health Insurance Portability and Accountability Act: the US law that gives patients a right to copies of their records and limits how providers and insurers use them.
  • DTI: Data Trust Index: SuperTruth's framework for grading how trustworthy data is. Here it grades the evidence behind each score; it never changes a score.

Who holds the keys

  • You (Individual) 0. The person holds the keys.
  • You and the health system (Shared) 51. The person has real controls inside a government or provider system.
  • Doctors, hospitals and insurers (Institutional) 81. Doctors, hospitals and insurers decide.
  • The government (State) 66. The government decides, with limited say for the person.

Our bar for "you hold the keys" is strict. You hold or direct the record. Nothing goes into a state or provider system unless you say so. Every share needs your OK first, and you can take it back. You can read a log of who looked. No country meets it today.

Known limits

  • Scores come from separate research passes against one set of scoring rules. An audit found that the passes sometimes scored the same legal situation differently in different regions. On October 1, 2026 we brought those scores onto the same rules, so no score is under review in this version. A score under review carries an 'Under review' tag on its country page and in the quick view.
  • Confidence: 27 high, 105 medium, 66 low. Confidence counts how many of the 8 rights rest on an official source, like a law, a government or regulator page, or an international agency such as WHO or OECD, with every key fact checked. High: 7 or 8 do. Low: 4 or fewer do. Medium: the rest. The evidence grade (DTI™) rates all the sources we cite, taken together, so a country can cite good sources overall and still have thin evidence on some rights. Both describe our evidence, not the country, and neither changes the score. Typical score by label: 48 out of 100 for high, 37 for medium and 40 for low; low-confidence countries appear in the Poor, Weak and Mixed bands. The label is about the evidence, not the country's rights.
  • Some laws are cited from secondary summaries rather than official text.
  • Treat two scores less than 5 points apart as the same.
  • Which places we rate. All 193 UN member states are rated, plus Greenland, Kosovo, Palestine, Taiwan and Vatican City: every UN member state, plus the places that run their own health systems and health record laws. The 8 other places on the map (territories of other countries, disputed areas and Antarctica) are not rated in this version.

About the real cases

Everything in this index comes from public information: laws, government and regulator pages, court decisions and published news reports. We link to every source.

These are published accounts from people and public bodies about problems with health records, pinned to the country where they happened. The summaries are ours; follow each link to read the original. We do not name the people, we include no one under 18, and we name a hospital or company only where a regulator, court or the organization itself has stated what happened. Real cases do not affect any country's score.

We searched news, regulator and ombudsman decisions, court rulings, parliamentary records and patient groups, in English and local languages. We did not use social media or forums. These are the accounts we could find and publish under these rules. They are not a sample.

SuperTruth sells health data verification products. We built this index and chose these cases ourselves; no one paid to be included or left out.
